Leave Donation Broadcast Request within DPW - Cindy McKenney

The below employee did not receive sufficient leave donations from his own facility to cover his absence, which is approved for leave donation. Consistent with the policy, this broadcast message is being sent to all employees within DPW.

Supervisors should share this information with employees who work within DPW, who do not have computer or e-mail access, in accordance with agency procedures.

Cindy McKenney, an employee of the Department of Public Welfare, who works with the White Haven Center State Hospital, is in need of 59 days of leave donations due to an approved medical condition. If you are interested in donating from one to five days of your accrued annual or personal leave and your remaining annual actual quota will be no less than five days after donation, please complete and sign the “Request to Donate Leave” form by October 30, 2009, and forward it to:
